What it means: The Municipal Corporation has no record of the birth. This usually happens because the hospital never notified them.

Fix:

  1. Contact the hospital where the birth occurred and ask if they notified the Municipal Corporation. Get a written confirmation or acknowledgement.
  2. If the hospital confirms it was notified but the record is missing, escalate to the ward CFC office with the hospital’s confirmation letter.
  3. If the hospital never notified, you must apply for late birth registration with hospital records as proof.

Rejection Reason 2: Hospital Records Required (Home Birth Misclassified)

What it means: You applied for a home birth but selected “Hospital Birth” on the form, so the system expects hospital records.

Fix:

  1. Reapply and select “Home Birth” or “Non-Institutional Birth” on the application
  2. Submit:
    • Statement from head of household
    • ASHA worker’s certificate (if available)
    • Statement from a midwife / Traditional Birth Attendant (Dai)
    • Address proof of place of birth

Rejection Reason 6: Father’s / Mother’s Aadhaar Not Linked to Mobile

What it means: OTP verification of parent’s Aadhaar failed because the linked mobile number is wrong or inactive.

  1. Visit the nearest Aadhaar Seva Kendra
  2. Update or link the active mobile number to Aadhaar
  3. Wait 24 hours and reapply

Rejection Reason 7: Late Registration Without Magistrate Order

What it means: The birth is more than 1 year old, but you applied through the regular online process without the required magistrate’s order.

  1. Apply for late birth registration at the Tehsildar / Authorised Magistrate office

Rejection Reason 8: Date of Birth Inconsistency Across Documents

What it means: The DOB you stated does not match the DOB on supporting documents (hospital records, school TC, Aadhaar).

  1. Identify which document has the correct DOB — usually the hospital record (closest to actual birth)
  2. Use that DOB in the application
  3. If the other documents have a wrong DOB, plan to correct them after the birth certificate is issued

Rejection Reason 9: Address Proof Doesn’t Match Place of Birth

What it means: The address proof you submitted does not show that you (or your parents) resided in the jurisdiction at the time of birth.

  1. Submit an address proof from the year of birth — old ration card, old electricity bill, old rent agreement
  2. If unavailable, submit an affidavit explaining your address at the time of birth
  3. Supporting documents like a vaccination card from a local PHC, or a school record showing the address, also help

Rejection Reason 10: Application Fields Missing or Incomplete

What it means: One or more mandatory fields in Form 1 (Birth Registration Form) were left blank or filled incorrectly.

Fix:

  1. Download the rejected application copy
  2. Identify all blank or incorrectly filled fields
  3. Fill every field, even if you have to write “Not Applicable” or “Unknown”
  4. Pay special attention to: time of birth, place of birth (full address), parents’ full names (no initials), and address

Rejection Reason 11: Affidavit Not Notarised or Wrong Format

What it means: You submitted an affidavit, but it was not on stamp paper, not notarised, or in the wrong format.

  1. Use the standard affidavit format
  2. Print on ₹100 stamp paper (or ₹500 for major corrections)

What it means: Your payment did not go through, or the system did not generate a receipt for the fee.

  3. Use a different payment method (UPI, card, netbanking) if the first one keeps failing
  4. Take a screenshot of every payment attempt
