How to Use These Templates
Step 1: Choose the Right Template
- Minor correction → Notary template
- Major correction (wrong DOB, wrong parent’s name) → Magistrate template
Step 2: Fill in Your Details
- Replace every blank with your actual information
- Make sure dates, names, and registration numbers are accurate
- Do not leave any blank unfilled — write “Not Applicable” if needed
Step 3: Get the Stamp Paper
- Visit a licensed stamp vendor (usually near the Tehsildar office or court)
- Buy ₹100 stamp paper (minor) or ₹500 stamp paper (major)
- E-stamp paper is also valid — available at SHCIL e-stamp centres
Step 4: Print the Affidavit
- Print the filled template on the stamp paper (or attach printed pages to the stamp paper with the front sheet stapled)
- Read it once more for accuracy
- Do not sign yet
Step 5: Visit Notary or Magistrate
- Notary: Visit any registered Notary Public — available near every court and tehsildar office. Cost: ₹100–₹300 for stamping.
- Magistrate (JMFC): Visit the local Judicial Magistrate First Class court complex. The clerk will help you book a hearing slot. Cost: stamp paper + ₹50–₹200 court fee. A lawyer is not mandatory but helps if you are unfamiliar with court procedure.
Step 6: Sign in Front of the Notary / Magistrate
- Show your Aadhaar card or other photo ID
- Sign the affidavit in their presence
- The Notary stamps it; the Magistrate signs it and applies the court seal
Step 7: Make Photocopies
- Make at least 3 photocopies of the notarised / magistrate-signed affidavit
- Keep the original safely; submit photocopies wherever required
Submitting the Affidavit for Correction
Once your affidavit is ready, submit it along with:
- Original birth certificate (or certified copy)
- Birth Certificate Correction Application (Form 8 or equivalent)
- All supporting documents mentioned in the affidavit
- Application fee (₹20–₹100)
at the same office that issued the birth certificate (Municipal Corporation / Gram Panchayat).
The Registrar may call you for verification. Once approved, the corrected certificate is issued in 15–60 days depending on correction type.

Important Tips
- Do not sign the affidavit at home. Notarisation requires you to sign in front of the Notary / Magistrate.
- Match your details with supporting documents. If your affidavit says one name but your Aadhaar says another, you may need a second affidavit to bridge them.
- Keep your supporting documents original. The Magistrate may want to see the school TC and hospital records in person.
- Use the exact spelling. The corrected name in your affidavit must match exactly what you want on the corrected birth certificate.
- Family members may need separate affidavits. If a parent’s name is being corrected, the parent themselves should also sign an affidavit confirming the correct spelling.
